Important Points on National Health Insurance (NHI)
Enrollment
- Those who have registered their residence in Kyoto City (or in other municipalities of Japan) are, in principle, required to enroll in NHI, excluding those who have enrolled in any employee insurance plan.
Certificate
- A “Health Insurance Eligibility Certificate (資格確認書)” is issued to those who have enrolled in NHI. Please be sure to bring it with you whenever you receive any healthcare services at a medical institution: by presenting the Certificate at the reception window of such medical institution, your copayment of medical costs will be 30%. (Those international students who visit Sakyo Ward Office by a group appointment are required to pick up without fail before you go home your Health Insurance Eligibility Certificates which will be issued on the day of their visit.)
Renewal of Certificate
- The Health Insurance Eligibility Certificate is automatically renewed/reissued in a different color every November. After you receive a new certificate sent by mail at home, dispose of the old one by yourself. In case you submit a NHI Enrollment Notification (加入届) in the month of October, you will receive a Health Insurance Eligibility Certificate which is valid until November of the following year, and there will be no renewal in November of the year of enrollment.
Insurance Premium
- NHI premiums (保険料) can be paid at either a convenience store, a branch of Japan Post Bank (Six prefectures in the Kinki region; Osaka, Kyoto, Hyogo, Wakayama, Nara and Shiga) or any other designated financial institutions with the payment slips which will be sent to you around the 20th of the following month of enrollment (in case you enroll in April, the payment slips will be sent to you two months later, in June). Note: As from Sept. 1, 2025, the cashless payment via smartphone app (PayPay, PayB, FamiPay, auPAY, and dPayment by Docomo) will be available).
- Your NHI premium amount shall be determined based on your total income earned in Japan in the previous year (your income earned outside of Japan, will not be included in the calculation.) For those who register their residence for the first time after migrating from overseas and did not have any income earned in Japan in the previous year, the premium amount for them is expected to be around 1,600 JPY/month. If your annual income from a part-time job is more than 430,000JPY/year (in case of salary income, more than 980,000JPY/year), your insurance premium amount to be paid as from April of the following year shall be increased according to the amount of your total income earned in the previous year.
- Since an “Income Tax Return Form (所得申告書)” shall be sent to your home around March every year, please be sure to fill it out and return it to us. If you do not submit the “Income Tax Return Form” duly filled, your even if you had no income in the previous year.
